Case Manager, RN

Rangely District Hospital has an opening in Nursing for a Part-Time Case Manager (RN) working 24 hours per week.

            Work Schedule:

·         Standard: 2 days per week, working this position Tuesday & Wednesday.

  • Flexible hours between 6 AM - 8 PM
  • Present for standard care conference times each week, Tuesday and Wednesday, from 1200-1700.

 

Job Statement:

 

The Case Manager is responsible for coordinating comprehensive care for patients within the nursing department.

Job Duties

  1. Care Coordination:
    1. Attend all care conferences for patients.
    2. Assist providers and staff nurses with care coordination and discharge planning based on patient needs and coordinate with outside agencies to ensure patients have appropriate resources.
    3. Ensure all inpatients have follow-up appointments scheduled prior to discharge.
    4. Track and ensure completion of SDOH screeners for all admitted patients.
    5. Enter primary care provider (PCP) information into Athena so that care summaries are sent.
    6. Submit PASRRs for patients as needed per regulations.
  2. Patient and Family Support:
    1. Facilitate e Consults and tele visits for inpatients and ER patients.
    2. Assist patients with miscellaneous tasks such as managing mail and attending internal appointments.
    3. Attend resident council meetings to address concerns and requests.
  3. Administrative Duties:

a.      Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.

b.      Evaluates outcomes of patient care, consults with other specialists as required and adjusts nursing care processes as necessary to ensure optimal patient care.

c.       Maintains patient documentation and assures the patients medical record is accurate and complete.

d.      Assists physicians with examinations, procedures and other processes related to direct patient care.

e.      Attends continuing education and in services to enhance professional growth.

f.        Maintains a safe, comfortable and therapeutic environment for patients/families in accordance with hospital standards.

g.      Supports and actively participates in departmental QA activities.

h.      Observes hospital’s compliance requirements does not create an issue of fraud and abuse for the hospital.

i.        Complies with all established hospital policies and procedures.

j.        Projects a positive image of the hospital through dealings with the public, patients, vendors and fellow employees by acting in a professional manner at all times.

 

Performs all other duties as assigned.

 

Education Requirements:

1.                  Graduate of an approved school of nursing (and, Diploma, ADN or BSN).

2.                  Current RN Colorado State Licensure.

3.                  Current and maintain CPR, ACLS, PALS, and TNCC.

Position Type: Part-Time

Shift: Tuesday and Wednesday

Hourly Wage Range: $29.60-40.84
